Understanding and Relying On the ASQ-3 Summary Sheet


The ASQ-3 Summary Sheet provides a straightforward way to determine those children who should be referred, those who should be monitored carefully and those whose development appears to be proceeding without problem.

White Area
Results: Shows a child is typically developing.
Action: Provide developmental activities to continue growth and learning.

Gray Area
Results: Suggest a mild to moderate delay
Action: If they are 0-3 years old-refer them to Early On.  If they are 3-5 years old-provide additional developmental activities and monitor their development by re-screening at the next available interval.

Black Area
Results: Suggest a potential significant delay
Action: Recommend referral to Early On/Project Find
